How do I delete my QualityHealth account?

To submit your request for account deletion:

  1. Click the "Submit A Request" button above.
  2. Enter your request into the form, selecting "Managing Your Quality Health Account" and then "Account Deletion".
  3. Submit your request.

 Our team will process your request within 10 days.
